DaVita Integrated Kidney Care (IKC) is seeking a Full-Time RN Case Manager (RN) to join the Port Charlotte, FL region. You will support complex patients, helping them navigate the healthcare system. Through medical record review and consultation, you will identify medical, social, emotional, and financial needs of patients with CKD/ESKD and implement appropriate interventions. You’ll work autonomously and with the care team to coordinate and facilitate quality, cost-effective care.

Position Details Location: Hybrid-Remote. Work from home (telehealth) and travel within an assigned geography for in-person visits (dialysis clinics and nephrology practices). Schedule: Monday – Friday; flexible to accommodate patient and nephrology partner availability. Employment type: Full-time. Key responsibilities: build empathetic relationships with patients and families; coordinate care including transitions and management of complex/at-risk patients; establish treatment plans with the care team; identify and address social determinants of health (medical, emotional, educational, community, financial concerns); coordinate for streamlined transitions of care; assess patients for unreported conditions or changes that may lead to adverse outcomes. Qualifications Current RN License required; BSN preferred. Minimum of 5 years RN experience required. Current CPR or BLS certification. Current driver’s license and willingness to travel within an assigned territory. Intermediate computer skills; proficiency in MS Word, Excel & Outlook. Home office with internet connectivity: minimum 1 Mbps upload and 1 Mbps download. Preferred Qualifications 2+ years of Case Management or Chronic Care Management – highly preferred. Compensation

The salary range for the role is $68,400.00 – $100,400.00 per year. Compensation depends on qualifications, skills, competencies and experience.
