Responsibilities
The social worker will conduct assessment to determine patient needs and provide support by developing and implementing individualized treatment.
The social worker will provide counseling and support to patients and their families, focusing on accessing additional resources and addressing mental health needs. The social worker with monitor and evaluate patient progress and adjust treatment plans as necessary. They will collaborate with multidisciplinary team to provide comprehensive and passionate care.
The provider will serve as a role model for Mercy’s mission and values-is caring, ethical, fair, respectable, compassionate, courteous and hospitable.
Requirements
Minimum of LSWA, LGSW or LCSW. LCSW-C preferredPreferred skill set includes:Accurate verbal and written expressionWork organization, problem solving Psycho-social counselingConflict resolution Identification of crisis situationsExperience with Bio-psychosocial assessmentsExperience with Crisis counselingExperience with Community resource referralsExperience with DocumentationMust possess a Bachelors or Masters Degree in Social WorkFluency in Spanish preferred
