RN Case Manager
Location: Washington, DC
Schedule: Monday–Friday, 8:00 AM–5:00 PM 40 hours/week
Pay Rate: $36.32–$50.00/hour
Position Overview
Under the direction of the Supervisory Care Manager, the RN Case Manager is responsible for providing comprehensive care coordination services to medically fragile children and young adults enrolled in the health plan. The RN Case Manager supports members with complex medical, behavioral health, and psychosocial needs by conducting assessments, developing care plans, coordinating services, and addressing barriers to care.
Caseload Structure
Standard RN Case Manager Caseload: Approximately 75 members Mix of Level 1, Level 2, and Level 3 acuity membersSpecialty RN Case Manager Caseload: Approximately 60 members Primarily high-complexity Level 3 membersAcuity Levels
Level 1: Stable, well-managed members requiring minimal intervention
Level 2: Moderate healthcare utilization and support needs
Level 3: High-risk, high-utilization members requiring intensive care management
Field & Office Expectations
Approximately 3 field visits per weekRemaining time spent completing documentation and care coordinationFlexible scheduling for field visitsTypically 4 days per week in the field and/or officeOpportunity to attend member well-child visits and provider appointmentsRequired Qualifications
Active DC Registered Nurse (RN) LicenseAssociate Degree in Nursing or higherMinimum 2 years of Case Management experience preferred
