Work Schedule: Day shift. This is a direct care opportunity to provide support for mental health treatment in Sheppard Pratt’s Segue Residential Transition Program. This position will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for your clients as necessary.
