Social Service Specialist

Sea Mar Community Health Centers, a federally qualified health center, is hiring a Social Service Specialist to provide comprehensive psychosocial assessment, brief counseling, and coordinated case management for clients.

ResponsibilitiesConduct integrated psychosocial assessments and develop intervention plans.Provide brief culturally relevant counseling (crisis intervention, mental health, parenting, etc.).Facilitate referrals to community agencies and assist clients in accessing services.Advocate on behalf of clients to resolve barriers (transport, language, etc.).Maintain accurate documentation in Sea Mar’s electronic health record system.Participate in interdisciplinary team meetings and provide input on care plans.Report suspected child or elder abuse as mandated by state law.Support infant case management and transition services for long‑term counseling.Observe productivity expectation of 4–5 patients per day.QualificationsMinimum of a baccalaureate degree in counseling, social work, or related field.Current certification as a counselor (State of Washington) or equivalent.Experience working with underserved populations, including limited English speakers and homeless clients.Knowledge of evidence‑based screening tools (PHQ-9, GAD-7, AUDIT, DAST, etc.).Strong verbal and written communication skills; bilingual English/Spanish preferred.Knowledge of the patient‑centered health home model and inter‑disciplinary care teams.Proficiency with electronic health record systems and Microsoft Office suite.Valid Washington State driver’s license; willingness to undergo background check, TB test, and obtain BLS CPR.BenefitsHourly rate of $27.71 plus benefits for eligible full‑time employees, including medical, dental, vision, prescription coverage, life insurance, long‑term disability, EAP, paid time off, holidays, and 401(k) retirement plan.
